Jill Hawkins was last seen Monday after telling family members that she was planning to visit her mother in Milwaukee. She never arrived.
Hawkins left behind her cell phone, according to her husband, Steve Hawkins, who says his wife may be suffering from depression.
Steve Hawkins is the entertainment manager for Route 20 Outhouse in Ives Grove.
Jill Hawkins was last seen wearing jeans and a short sleeve brown sweater. She has a tatoo on her left arm.
She drives a VW Passat with a license plate of 559XJB.
